Children’s Medical Report

Please note that each child is required to have a physical

at the beginning of each school year signed by a licensed physician.

It is due within 30 days of the start of school.

In Case Your Child Is Ill:

Children that are obviously ill upon arrival in the morning will not be allowed to stay. Children who become ill will be isolated and the parents will be notified to pick them up as soon as possible. We realize that all illness cannot be prevented and that during the Preschool years many children are exposed to communicable diseases. However, we have a responsibility to each child in our Preschool and feel that if good health and hygiene are followed, the spread of many of these diseases can be kept to a minimum.

Please Do Not Bring Your Child If He/She:

Has a fever of 99 degrees or higher(Must be fever free for 24hrs without meds
Has been vomiting or has had diarrhea within the last 24 hours
Has matted, yellowish discharge from eyes or has pinkeye
Has fresh cold (colored drainage from the nose) deep cough, or sore throat
Has a communicable disease such as measles, mumps, chickenpox,

strep throat, impetigo, flu, roseola, hand-foot-mouth disease
If your child has any of the following symptoms Please contact their Doctor as these could be symptoms of Covid-19
Fever or chills
Cough
Shortness of breath or difficulty breathing
Fatigue
Muscle or body aches
Headache
New loss of taste or smell
Sore throat
Congestion or runny nose
Nausea or vomiting
Diarrhea

First Aid

When children are at play, scratches and bumps can happen.

Such injuries will be cleansed and treated with first aid cream and band-aid.

In the event that a more serious incident occurs, you will be notified immediately.

Snack

Your child will be provided a nutritious snack each school day.

Please let us know if your child has any food allergies.
